Guides / Weight Loss – Wegovy (Semaglutide)
Everything you need to know about semaglutide (Wegovy) weight loss treatment.
Understanding Wegovy
1. What is semaglutide (Wegovy)?
Semaglutide, marketed as Wegovy for weight management, is a once-weekly injectable prescription medication that mimics the naturally occurring gut hormone GLP-1 (glucagon-like peptide-1). It was originally developed for type 2 diabetes under the brand name Ozempic and was subsequently approved at higher doses specifically for weight loss.
Wegovy is approved by the MHRA for chronic weight management in adults with a BMI of 30 or above, or 27 or above with at least one weight-related comorbidity. It represents one of the most significant advances in medical weight management in decades.
2. How does Wegovy work?
Wegovy works through three complementary mechanisms that target different parts of the body:
- Brain (appetite centre) — acts on GLP-1 receptors in the hypothalamus to reduce hunger signals and increase feelings of satiety, fundamentally changing your relationship with food
- Stomach (digestion) — slows gastric emptying so food stays in the stomach longer, keeping you feeling full for extended periods after meals
- Pancreas (insulin) — improves insulin secretion and sensitivity, helping your body process glucose more efficiently and reducing insulin-driven fat storage
Together, these effects reduce caloric intake naturally without the constant hunger that undermines traditional dieting.
3. Am I eligible for Wegovy?
Wegovy is prescribed for adults with a BMI of 30 or above (clinically obese), or a BMI of 27 or above with at least one weight-related comorbidity such as type 2 diabetes, high blood pressure, obstructive sleep apnoea, or cardiovascular disease.
You must also be willing to make concurrent lifestyle changes including dietary improvements and increased physical activity. Wegovy is not intended for cosmetic weight loss in individuals at a healthy weight. Your Heracles clinician will assess your full medical history to determine suitability.
4. How effective is Wegovy for weight loss?
The landmark STEP-1 trial demonstrated that participants taking semaglutide 2.4mg lost an average of 15% of their body weight after 68 weeks, compared to 2.4% in the placebo group. One-third of participants lost more than 20% of their body weight.
For a 100kg man, this translates to approximately 15kg of weight loss. Results vary between individuals, and outcomes are significantly enhanced when medication is combined with structured diet, exercise, and behavioural modifications.
5. What are the health benefits beyond weight loss?
The benefits of 10–15% body weight loss extend far beyond aesthetics. Evidence-based benefits include:
- Blood sugar control — improved HbA1c and insulin sensitivity, reducing type 2 diabetes risk
- Cardiovascular health — the SELECT trial showed a 20% reduction in major cardiovascular events; improvements in blood pressure and cholesterol
- Visceral fat reduction — preferential loss of harmful abdominal fat that surrounds organs
- Hormonal improvement — reduced aromatisation means more free testosterone; improved erectile function and libido
- Sleep quality — reduced sleep apnoea severity and improved sleep architecture
Many men report improvements in energy, confidence, and mental clarity that transform their daily quality of life.
Safety & Side Effects
6. What are the common side effects?
The most common side effects are gastrointestinal and tend to occur during dose escalation, improving as your body adjusts. These include nausea (the most frequent, affecting around 20–30% of patients), diarrhoea, constipation, vomiting, and abdominal discomfort.
Most GI side effects are mild to moderate and self-limiting. Eating smaller meals, avoiding fatty or spicy foods, staying hydrated, and eating slowly can help manage symptoms. Your clinician can slow the dose escalation if side effects are troublesome. Serious side effects are rare.
7. Who should not take Wegovy?
Wegovy is contraindicated in individuals with:
- Multiple Endocrine Neoplasia syndrome type 2 (MEN-2) — a personal or family history
- Medullary thyroid carcinoma (MTC) — a personal or family history; GLP-1 agonists caused thyroid C-cell tumours in rodent studies
- Type 1 diabetes — Wegovy is not a treatment for type 1 diabetes and could cause dangerous blood sugar fluctuations
- History of pancreatitis — or active gallbladder disease
- Known hypersensitivity to semaglutide or any excipients
A thorough clinical assessment is conducted before prescribing to identify any contraindications. Caution is also advised in patients with severe renal impairment or a history of diabetic retinopathy.
17. Can I drink alcohol while taking Wegovy?
There is no absolute contraindication, but alcohol should be limited. It provides empty calories (7 calories per gram), can worsen GI side effects like nausea, and may lower blood sugar levels when combined with semaglutide. Many patients also report a naturally reduced desire for alcohol while on treatment.
If you choose to drink, stick to moderate amounts — ideally under 14 units per week. Avoid binge drinking and be aware that your tolerance may be lower than usual. Minimising alcohol will improve your weight loss results and overall health outcomes.
Treatment
8. What is the Heracles approach to prescribing Wegovy?
At Heracles, we take a thorough, clinician-led approach. The process begins with a detailed health questionnaire covering your medical history, weight history, medications, and goals. You will submit recent photos for clinical assessment of body composition.
Your clinician may recommend optional blood tests to check baseline metabolic markers, thyroid function, and kidney health. Once approved, you receive a personalised treatment plan with ongoing clinical support, dose adjustments, and regular reviews. Medication is dispatched directly to your home.
9. How do I take Wegovy?
Wegovy is administered using a FlexTouch pre-filled pen. You inject once weekly into the abdomen, thigh, or upper arm. The pen has a hidden needle that you do not see before or during injection, making it comfortable even for those nervous about needles.
To inject: select your dose, place the pen flat against your skin, press and hold the dose button until the dose counter shows 0, then count to 6 before removing. Inject on the same day each week and rotate injection sites. Detailed step-by-step instructions are provided with your first delivery.
10. What is the dose escalation schedule?
Wegovy follows a structured 16-week dose escalation to minimise side effects:
- Weeks 1–4: 0.25mg weekly
- Weeks 5–8: 0.5mg weekly
- Weeks 9–12: 1.0mg weekly
- Weeks 13–16: 1.7mg weekly
- Week 17 onwards: 2.4mg weekly (maintenance dose)
The gradual increase allows your body to adjust and significantly reduces the severity of GI side effects. If side effects are troublesome at any stage, your clinician may extend the time at a lower dose before escalating further.
11. What is the realistic timeline for results?
Weight loss begins within the first few weeks of treatment as appetite decreases. The timeline for meaningful results follows a predictable pattern:
- Month 1–2: noticeable appetite reduction; initial weight loss of 1–3%
- 3–6 months: most patients achieve 5–10% body weight loss; visible changes in body composition and waist circumference
- 12–15 months: maximum weight loss of 10–15% at the maintenance dose; significant improvements across all health markers
Weight loss is not linear. Plateaus are normal and do not mean the medication has stopped working. Your clinician will monitor progress and adjust the plan accordingly.
18. How long should I take Wegovy, and what happens when I stop?
Most men use Wegovy for 12–18 months to reach their target weight. Some may continue on a maintenance dose long-term if clinically appropriate. The decision to continue or stop is made collaboratively between you and your clinician based on your progress and goals.
When semaglutide is discontinued, appetite gradually returns to baseline over several weeks, and studies show that around two-thirds of weight lost may be regained within a year without sustained lifestyle changes. This underscores the importance of using the treatment period to establish lasting diet, exercise, and behavioural habits.
Lifestyle & Optimisation
12. How can I maximise my results on Wegovy?
The four pillars that maximise weight loss outcomes are:
- Protein-rich diet — aim for 1.6–2.2g protein per kg body weight daily to preserve lean muscle mass; prioritise protein at every meal
- Strength training — 2–3 sessions per week of resistance exercise to maintain muscle, boost metabolism, and improve body composition
- Quality sleep — 7–9 hours nightly; poor sleep increases hunger hormones and impairs fat loss
- Stress management — chronic stress elevates cortisol, which promotes visceral fat storage and undermines weight loss efforts
Men who combine Wegovy with these lifestyle modifications consistently achieve better results than those relying on medication alone.
13. What should I eat while taking Wegovy?
Because Wegovy reduces appetite, the quality of what you eat becomes even more important — every calorie needs to count. Focus on protein-first meals with lean meats, fish, eggs, dairy, and legumes. Include plenty of vegetables, healthy fats (olive oil, avocado, nuts), and complex carbohydrates.
Avoid ultra-processed foods, sugary drinks, and excessive refined carbohydrates. Eat slowly and stop when satisfied — the medication will help you recognise fullness signals more easily. Stay well hydrated with at least 2 litres of water daily, as dehydration can worsen nausea and constipation.
14. What exercise should I do alongside Wegovy?
Resistance training is essential. During any significant weight loss, some muscle mass is inevitably lost alongside fat. Strength training is the most effective way to minimise this, preserving the metabolically active tissue that keeps your resting metabolic rate high.
Combine 2–3 resistance sessions per week with at least 150 minutes of moderate-intensity cardiovascular exercise (brisk walking, cycling, swimming). If you are new to exercise, start gradually and build up. Even daily walking makes a measurable difference. The goal is consistency, not intensity.
15. How do I prevent losing muscle while on Wegovy?
Muscle preservation requires a two-pronged approach: high protein intake and resistance exercise. Without these interventions, up to 25–40% of weight lost may come from lean tissue. With them, you can shift the ratio so that 80–90% or more of weight lost is fat.
Distribute protein intake evenly across meals (30–40g per meal). Include a protein source within 1–2 hours after resistance training. If appetite is very suppressed, protein shakes or Greek yoghurt can help you reach your daily target without requiring large meal volumes.
16. Will Wegovy improve my testosterone levels?
Wegovy does not directly affect testosterone production, but the weight loss it produces can significantly boost testosterone indirectly. Excess body fat — particularly visceral fat — increases aromatase activity, an enzyme that converts testosterone to estrogen. Losing weight reduces this conversion.
Studies show that a 10% reduction in body weight can increase testosterone levels by 50–100 ng/dL or more. Men who are overweight with borderline low testosterone may find that weight loss alone brings their levels back into the normal range, potentially avoiding the need for TRT.
19. How important is mindset for long-term success?
Mindset is arguably the most underrated factor in successful weight management. View Wegovy as a tool that gives you a window of opportunity — it makes the biological aspects of weight loss easier, but the psychological and behavioural work is what sustains results long-term.
Set goals beyond the number on the scale: how you want to feel, what activities you want to enjoy, how you want your health markers to look. Track non-scale victories like energy, strength, sleep quality, and confidence. Accept that progress is not linear, and treat setbacks as data rather than failure.
